Yes, I would like to come in as soon as possible to collect the paperwork you still have so we can add more recent stuff to it and then post it ourselves.

 

Can you please write down the specific address for Centre for Health and Disability Assessments (CHDA) in full that I should be sending this to thank you

Can you also write down the specific address where the gp in question operates from, but if you don’t know then please let me know thanx.

Can you also write down the address for Victoria at the Lowestoft Service Centre as I cannot find it anywhere, but if you don’t know please let me know again thanx.

Also, who is the overall person in charge of the two uc job centres in Redditch. If there is a person in charge I would like that name so please write that down too thanx.

 

 

John is at school so I can come down right away if you can get it all ready. I pick him up between 2.15pm and 3.15pm. I drop him off and am done by 9.15pm each morning. I can come down any day of the week if that’s easier for you.

You can just give it to the staff at the door if you wish, but please make sure you attach a piece of paper with the above questions answered to the best of your ability, or put them on the journal.

I would like you to know that I have decided to have the injection Zoladex and next week I will be making an appointment to see my GP about getting the injection started and to have a re-assessment on my current situation.

 

I would like you to know that I did receive my appointment letter to see you on Friday 1st April 2022 at 10am at Worcestershire Royal Hospital. I would like you to know that I will be attending this appointment and that my mother in law will be taking me. Unfortunately my partner Stuart will not be able to come to this appointment as he has to take our son to school.

Hi again DWP.

It has now been made clear to me that the DWP are solely responsible for the hiring and outsourcing to the GPs in question as per the quote below from a UC employee as follows…

‘’I am unable to answer most of your questions. I am unaware of whom you would need to speak to regarding Legal Aid or to make a complaint about a GP. The GPs which conduct the work capability assessments do so for the agency outsourced by DWP to do so.’’ ((Scott Wrexham Service Centre))


So please please please can the DWP now enlighten me as to how to make a formal written complaint against a person you have outsourced/employed to act on your behalf as a GP (Mr Mohammed Naiwaz) who I presume was acting on his oath to the patient at the time, and has and a license to practice I am requesting a copy of here in case the GMC need informing, as well as a person employed within the assessment outsourced/employed departments of the DWP ((
Victoria at the Lowestoft Service Centre)) who has avoided and failed to respond or reply to my emmas concerns or grievances, and we still have not been told by her if she actually received the recorded delivery bulk of documents she requested, that the UC refused to accept on site???

And after a recent visit to the job centre, they again refused to accept or scan a recent documents of emmas correspondences with the surgeon, due to data protection, and myself being told not to upload documents to the journal a while back due to the same reason, how are we to inform you of our circumstances.

 

Legal Aid. Simple question that no-one at uc can answer. How do we get legal aid. Who do I go to to get legal aid or see if we are entitled. Does the dwp know where we can go to find out about legal aid please.

Put simply. To me the above is criminal negligence at the very least. Im saying a crime has been committed. What do I do to get this investigated properly and formally please dwp.

Do I need to go to the police station. Is there an ombudsman. Do the dwp have a complaints procedure for this scenario or are we the very first man and woman ever to raise this issue against a person.

Please treat this as an FOI request (freedom of information request act) if it will help you actualise and realise the weight of our grievance and concern that I have now formally enlightened you to.

this is just a small plea to let you know I have been trying to get my local Redditch job centre to acknowledge our formal complaint for three weeks or so now, regarding their GPs evaluation of my partner emma jane Goode’s current chronic health condition. They have not responded at all to the formal complaint.

 

we are both accusing the GP of making things up or lying to be very honest with you, and my partner and i have already formally complained to the Redditch job centre in writing multiple times, and also now insisted they acknowledge the formal complaint as an FOI request where ever a question is asked or present. (FOI act freedom of information act)

A member of staff has also re-evaluated Emma since, and totally and utterly ignored our complaint. Initially the job centre even refused our paperwork when they insisted Emma come in, and universal credits were the ones that asked us to supply it in the first place!!!

 

It was posted recorded delivery on Tuesday 14th December 2021 to get it into Emma’s file, on the very same day, and they still haven’t confirmed receipt of this. Even when asked for confirmation through the online journals since too.

 

I even asked how I can make a formal complaint, and got no response whatsoever. Thank god for your wonderful website, and this is how and where I got your address, and why you are reading this now.

All recent info regarding Emma’s poor health has been included in this envelope for you to consider. There is much more has been posted to the job centre, as well as all out info posted on the journals.

Please please please can you help us to get some sort of response regarding our grievances, or at least advise us on where to go to next to get our complaint heard fairly.

3rd Letter To Doctors Tramadol

This letter is an update from the previous letter that I sent by recorded delivery on Tuesday 11th January 2022. I have received an email from St Stephens Medical Partnership on Tuesday 18th January 2022 saying that you are currently investigating my last letter I sent to you.

I sent an email to PRESCRIPTIONS, M81001 (NHS HEREFORDSHIRE AND WORCESTERSHIRE CCG) on Friday 14th January 2022 at 8.48am for a new repeat prescription of 60 Tramadol.

Later that day I received a text message from the surgery at 12.31pm which stated and I quote:

"Dear Mrs Goode

With regard to your recent prescription request, the GP has requested you contact the surgery by telephone only to book a GP telephone consultation to dis cuss this matter further.

Thank you, St Stephens Medical Partnership"

So I phoned the surgery at 1.30pm to book a telephone consultation about my recent prescription request. I was first told that I can have a telephone consultation on Monday morning. I explained how I thought it was about my recent prescription request and I needed a call this afternoon. I was then told a doctor will phone me between 3 and 4pm.

A doctor by the name of Dr Whasir phoned me at 3pm. She said that she was a locum doctor. Below are the notes from the telephone conversation we had:

Dr Whasir asked me what the call was for and how she can help me. She told me that she was told just to ring me and had no information on what the call was about.

I explained to her about the email I sent in the morning requesting for a new repeat prescription of 60 Tramadol. I then told her about the text message I got at 12.31pm.

Dr Whasir asked why I needed the Tramadol. I told her that I got Endometriosis and that I needed the Tramadol for the constant excruciating crippling stabbing pains I suffer with all day every day.

Dr Whasir told me that my last request of 60 Tramadol that I requested on Tuesday 4th January 2022 was rejected. Can you please tell me why this was rejected? NEED A REPLY TO THIS QUESTION.

I explained to Dr Whasir about all the hassle I was having with my Tramadol. I also explained that I needed 60 Tramadol and not 30 Tramadol.

Dr Whasir asked me how long 30 Tramadol would last me. I told her a week.

Dr Whasir then asked me how long 60 Tramadol would last me. I told her properly about 2 weeks.

Dr Whasir then asked how many Tramadol I take in a day. I told her sometimes its 4 but lately it has been 5.

Dr Whasir said she was working out the amount of Tramadol I take in a day. She then said that I was right about the amount of Tramadol I should be having and that I should be having 60 Tramadol and not 30 Tramadol.

Dr Whasir told me that she put my prescription back up to 60 Tramadol and sent it over to the Pharmacy. She explained that it will be ready to be collected on Saturday morning.

Dr Whasir explained about the side effects. I told her that out of all the medications I have had in the past, Tramadol is the only one that has never gave me any bad side effects.

You have now got an actual doctor stating that I should be having 60 Tramadol every time not 30 Tramadol. I would like this noted on my file and on your records for future reference.

Please can you make sure that my prescription of 60 Tramadol remains at 60? Unless we have a consultation and we both agree to reducing the amount down.

My partner Stuart went to the Pharmacy on Saturday 15th January 2022 and collected my prescription. I did receive my 60 Tramadol.

I would like to say a big thank you to Dr Whasir for her co-operation and understanding, as well as confirming and acknowledging that 60 Tramadol is a reasonable accepted amount for my current status and condition.

I would be very grateful if you can reply within next 7 days to both this letter and my last letter. Thank you for sending me an email telling me that you looking into my concerns regarding my Tramadol.

i was hoping legal aid might help us get gp or civil service accountability regarding emmas condition, as well as treatment. i didnt intend to use it for the jobcentre or jobcentre gp issue. so again, if theres help please let me know via url thank you. if legal aid doesnt apply to us in this context then please accept my appologies for the mistake on my part.(or derivatives of legal aid thereof)

No, in all honesty it was to do with your GP in question and victoria but as this has now been made clear to me by the manager at the job centre that an foi cannot be used in this context, i believe the FOI request should be cancelled and made null and void by myself. the foi request was intended to get a response from anyone at universal credits or dwp and it did, so please accept my appologies for the foi request and please assume it is no longer required thank you.

 

i instead want he matter to be dealt with as a formal complaint as originally declared and intended by myself and emma, BUT if you or the job centre can give me your assurances you will do everything in your reasonable best efforts to try to prevent this happening to anyone else again, then please deal with it as an internal matter that needs no more response from you to us except for maybe a small aknowledgment to us the matter has been dealt with. i did say this to the manager yesterday.

((we were disgusted and upset at the treatment of emma tbh, but yesterdays meeting has more than made up for that mistake now.))

if so, and theres nothing you can do at all internally, even though he technically was employed by yourself to act or deliberate on your behalf at the time, then i must ask for his full name and license number as a registered gp so i can take it up with the relevant governing authority. if you cannot deal with this matter internally and decisively, then i will take it up with the relevant governing body or whoever that is. please advise here if you know who. has this never happened before? if you cannot deal with this then who can? does dwp know who can deal with this? do you know who i got to next please?

 

 

 

ps. please thank the staff for yesterday, and let the wonderful lady know that emma and i actually made a realisation that we could hire a wheelchair and go out for an hour or two around the town. so something very positive happened as a consequence of yesterdays appointment. thank you very much. god bless you all.

Tramadol Prescription

On Tuesday 4th January 2022 my partner Stuart posted my letter to you in your prescription box inside the entrance. It was at about half 6/7pm. The letter explained and asked why my repeat prescription that I got on Wednesday 29th December 2021 was only 30 Tramadol and not 60 Tramadol.

I asked in the letter why was my prescription changed without my knowledge or consent? I also explained in the letter that this was the third time it had happened. I have had no response to this what so ever. Why did I not get a response to this?

In the letter I stated that I emailed for a new repeat prescription that morning for 60 Tramadol. In the email I stated that I would like 60 Tramadol and not 30 Tramadol like I had before. I asked in the letter for you to honor this request and you did not.

Stuart went to pick up my prescription on Wednesday 5th January 2022 and when he got back home he told me that it was only 30 Tramadol again and not 60 Tramadol like I asked for.

I have scanned both receipts from the pharmacy and the prescription slip that I got with the first lot of Tramadol from Wednesday 29th December 2021 (if you would like a copy please let me know). I noticed that a Dr R J Burling did the first prescription but I do not know who did the second one. Can you please let me know who did the second prescription (dated Wednesday 5th January 2022)? Thank you.

I would like to know why both of these prescriptions was reduced without my knowledge or informed consent? I would also like to know why I got no response from my last letter?

Why am I not getting my normal prescription of 60 Tramadol? My situation has not changed at all. I am still in constant excruciating crippling stabbing pain all across my abdomen and all along my right side abdomen. I am still bed ridden day in and day out.

I would be very grateful if you can please put my prescription back up to 60 Tramadol. Please speak to me in the future first about any medication on my file.

I would be very grateful if you can please answer all of my questions that I have asked above and get back to me as soon as you can thank you.

If this ever happens again I am going to do a formal complaint against you to your highest authority and get my partner Stuart to help me to do an F.O.I request.

 

Telephone Conversation

I stated in my last letter that I was going to have a telephone conversation on Friday 7th January 2022 with my Endometriosis Specialist Miss D Ghosh.

I have had the phone call but Miss Ghosh is on maternity leave for 9 months so I spoke to another Endometriosis Specialist by the name of Dr Natalie Brice. Attached with this letter is the notes I made from that Telephone Conversation. I am also will be getting notes from Dr Brice about the conversation we had on the phone.

On Saturday 8th January 2022 I received a Treatment advice note about the injection we talked about over the phone called Zoladex. I have scanned and kept a copy of this Treatment Advice Note and the original is attached with this letter for you to keep on my file.

I am going to be doing some research on this injection Zoladex before making up my mind on whether I want it or not. I will get back in touch with you within the next couple of weeks with my answer.

 

Book an Appointment

I stated in my last letter that in one of Miss Ghosh letters to me (dated Friday 19th November 2021) she stated that I should book an appointment with you face to face for a re-assessment on my condition and situation. I will be doing this within the next week or so. I have attached a copy of that letter to this letter.

I would like it noted on my file that I will be bringing my partner Stuart to this appointment and any other appointments I might have in the future. The reason for this is I would like him to know and listen to what is being said in the appointment and I would like him to give the doctor his side of things as well. Stuart also speaks on my behalf with my full consent as I struggle sometimes to understand what is being said to me.

Notes from Telephone Conversation - Friday 7th January 2022

 

My Endometriosis Specialist Miss D Ghosh has just had a baby and is on maternity leave for the next 9 months.

Spoke to Endometriosis Specialist Dr Natalie Brice. She said she was on her own while speaking to us. Me and Stuart was there on speaker phone.

Pains not getting any better. Still bed ridden. Still using crutches around the house and when I am out and about. Please look at my diary or ask for more information.

Stuart told her about my website www.nomoreendo.com, which has got all my information on and my pain diary. Dr Brice mentioned that she is not allowed to look at my website, so we promised to send her all the latest information on my current situation.

Dr Brice mentioned the next step was pain medication but both me and Stuart said that is not an option for me at all as all the pain medication I have had in the past did me no good and I had really bad side effects to them. The only pain medication that has really helped me is Tramadol. I would like to try Oramorph as my sister is on this but I was refused it by my GP.

Dr Brice mentioned about an injection called Zoladex, which stops my ovaries from working all together and stops hormone production and helps with the pain of the Endometriosis. She said that if the pain I am suffering with is caused by my Endometriosis then this injection will stop it completely if not then the pain is not caused by Endometriosis. She explained how this injection has had 90-100% success rate with other patients. Dr Brice mentioned that I have the injection once a month for 3 months and it needs to be administered by my GP. Dr Brice also said that if the injection works then they might do a hysterectomy which is what I have been asking for from day 1.

I mentioned that since day 1 I wanted to have a Hysterectomy but Dr Brice was not inclined to do that has it might not solve the problem. Dr Brice mentioned that there was no notes on my file mentioning me asking for a Hysterectomy. There are 2 reason’s why I am asking for a Hysterectomy 1) because I have heard so many women saying that they have had the Hysterectomy and it helped a lot with the pain, some of them have even said that they go back to their normal life pain free. 2) I recently found out that my sister (4 years old than me) who also suffer with Endometriosis is going to be having a Hysterectomy within the next 3-4 months. She was offered the Hysterectomy as soon as they found out she had Endometriosis but she declined it because she wanted to have another baby. Now that she got a baby she is now having the Hysterectomy. Every time I have asked for the Hysterectomy, from day 1 I have always been told no and that I am too young to have it. I want to have the Hysterectomy because I do believe that it will be the best thing for me and it will definitely help with the pain.

I said I will need to do some research on the injection and look into it more before making up mind on whether to have it or not. If I decide to have the injection I would start getting it within 2 weeks.

Dr Brice mentioned that she was looking at photos on the computer while speaking to us on the phone and that everything looks normal apart from the Endometriosis.

Stuart asked if there could be anything else that could be causing my pains in my right side abdomen as all of the Endometriosis was found in my left side and in the middle of my abdomen. Dr Brice mentioned muscle pains. Stuart mentioned about hyenas and incisions.

Dr Brice mentioned about seeing a chronic pain specialist or a physiotherapist.

I asked about other scans that can been done to see if there is anything else going on, rule out other things and for my own peace of mind. Dr Brice mentioned that they could do an MRI Scan, which scans the whole body.

Stuart asked about seeing Dr Brice face to face and she said properly within the next 3-4 months would be best and it would be best to see her after I had the injections. We both agree to this decision.

Dr Brice said if I needed to get in touch with her, to email her through Miss D Ghosh’s secretary Stephanie Lloyd. I need to let her know what my decision is on the injection when I have made it and to send all of the paperwork.

Stuart asked her to send us notes on what we discussed on the call and she agreed to do so.

complaint 2 FOI request

there are two parts to this update, and i will journal them seperately for conveniance.

 

GENERAL INFO

 with regards to legal aid or entitlements of other such schemes available to us, i will deal with that at a later date, seperately, and i will make a long overdue appointment to come in and see you personally. i wont be needing citezans advice at this present time. if you feel an appointment with me to come in and see you personally will be benneficial you are more than welcome to schedule one.

 

we did have steve over yesterday 5th jan 2022 from worcester council hub early help, to help with john at school and maybe even emma and her condition. we will keep you posted as necessary.

 

i went in to see Shopmobility in redditch town centre yesterday (Wednesday 5th January 2022) about booking a wheelchair hire for emmas appointment on 13th jan 2022 9.40am-10am. they did say i could use the service but i had to hire it on the day of the appointment and could not book it. so it is our every intention to come in and see you and honour your request and show willing for appointment but i just want you to be aware i couldnt book the wheelchair in advance. if theres any problems we will let you know immediately but i think all will be well on the day. and emma may be in some pain but atleast she doesnt have to walk it all so thats a great relief to me. all in all, emma will be there for her appointment if theres a wheelchair available. i will get emma to the door and wait outside as before, unless otherwise stated.

 

emmas surgeon is calling us tomorrow 07-01-2022 at 10.30am after the six month period wait. emma is clearly not recovering and i think she will be asking again about a hysterectomy. scares the life out of me but she hasnt got her life back yet and i cant make her better. her sister ironically has just been confirmed the exact same procedure within the last week, and i am desperately hoping she could be a yardstick of reference for emma so to speak.

MY FORMAL COMPLAINT

 

sad to say it has been nearly 2 weeks and still no confirmation of our formal complaint made against the gp (Mr Mohammed Naiwaz), and we still have no had no feedback at all whatsoever from Victoria at the Lowestoft Service Centre about the paperwork we sent first class recorded delivery on Tuesday 11th December 2021, and her recent re-assesment of emma dated Thursday 16th Decemeber 2021 .

 

my primary goal now is to get our formal complaint dealt with in amore proper, timely, constructive, meaningful manner.

 

 

 

MY FREEDOM OF INFORMATION REQUEST (FOI act)

 

please now treat our last update done by both of us dated Sunday 26th December 2021 about the complaint as a formal legal FREEDOM OF INFORMATION ACT REQUEST, and deal with any questions in there, under the said and proper act accordingly.

 

please now treat emmas previous response to the above, relating to the gp assesment and emmas grievances, and Victoria at the Lowestoft Service Centre more recent assesment, as a formal legal FREEDOM OF INFORMATION ACT REQUEST, and deal with any questions in there, in the proper way.

 

by law you are allowed 30 days to respond, but i will gladly give you more grace time if you so require, but could you please please please at least let us know as soon as possible, that this matter has now been passed on to your relevant legal team, foi ombudsman, or whoever deals with your foi requests and formal complaints now. if i need to post it to someone then please let me know.

 

if i dont hear back withing 5 working days, i will insist on a face to face appointment with you as soon as humanly possible at your discretion, and may consider getting paid legal advice if we can afford it. im sorry, but a member of your employment has lied, maybe more, and it needs dealing with promptly and properly and according to the law.

We (Emma and Stuart) would like to know why my Tramadol has been changed from 60 to 30 tablets, yet again, and without any contact or warning or discussion with me first. This was done without my consent. Surely you should at least talk to the patient before you start modifying the medications. This is the third time now, and we just want to know why this has been done thank you.

I have put in for a new repeat prescription for my 60 Tramadol via email this morning (Tuesday 4th January 2022). I would like you to honour this but if you cannot then please email me to let me know why.

I have got a telephone conversation with my Endometriosis Specialist Miss D Ghosh Friday at 10.30am. She has mentioned in a letter she sent to us that I should come in to see you to be reassessed by you about my condition. We will be in touch sometime in the next couple weeks to see you after we have spoken to the surgeon to help us in the right direction.

I am going to be brining my partner Stuart in with me so he can listen and give you his side of things.

Emma still not well, we need to know the next steps for the endo from Ghosh on the left side and in the middle of the abdomen as per surgeon, and maybe see someone else about the severe sharp pains still on right side by keyhole and ectopic procedure.
