Regarding our latest telephone conversation on Friday 7th January 2022. I have typed my notes from the conversation and below I have attached those notes to this email.

You asked for all our latest information, statements and letters that we sent out. They are mostly sent to the Universal Credits about my current health and situation but I think you should have a copy of them as it will tell you how bad my situation has got.

I would like you to know that I have decided to have the injection Zoladex and next week I will be making an appointment to see my GP about getting the injection started and to have a re-assessment on my current situation.

 

I would like you to know that I did receive my appointment letter to see you on Friday 1st April 2022 at 10am at Worcestershire Royal Hospital. I would like you to know that I will be attending this appointment and that my mother in law will be taking me. Unfortunately my partner Stuart will not be able to come to this appointment as he has to take our son to school.

 

Please let me know that you have received this email and all the attachments safely and if not we can post them to you.

DWP 2nd update regarding failure to respond to formal complaint

Hi again DWP.

It has now been made clear to me that the DWP are solely responsible for the hiring and outsourcing to the GPs in question as per the quote below from a UC employee as follows…

‘’I am unable to answer most of your questions. I am unaware of whom you would need to speak to regarding Legal Aid or to make a complaint about a GP. The GPs which conduct the work capability assessments do so for the agency outsourced by DWP to do so.’’ ((Scott Wrexham Service Centre))


So please please please can the DWP now enlighten me as to how to make a formal written complaint against a person you have outsourced/employed to act on your behalf as a GP (Mr Mohammed Naiwaz) who I presume was acting on his oath to the patient at the time, and has and a license to practice I am requesting a copy of here in case the GMC need informing, as well as a person employed within the assessment outsourced/employed departments of the DWP ((
Victoria at the Lowestoft Service Centre)) who has avoided and failed to respond or reply to my emmas concerns or grievances, and we still have not been told by her if she actually received the recorded delivery bulk of documents she requested, that the UC refused to accept on site???

And after a recent visit to the job centre, they again refused to accept or scan a recent documents of emmas correspondences with the surgeon, due to data protection, and myself being told not to upload documents to the journal a while back due to the same reason, how are we to inform you of our circumstances.

 

Legal Aid. Simple question that no-one at uc can answer. How do we get legal aid. Who do I go to to get legal aid or see if we are entitled. Does the dwp know where we can go to find out about legal aid please.

Put simply. To me the above is criminal negligence at the very least. Im saying a crime has been committed. What do I do to get this investigated properly and formally please dwp.

Do I need to go to the police station. Is there an ombudsman. Do the dwp have a complaints procedure for this scenario or are we the very first man and woman ever to raise this issue against a person.

Please treat this as an FOI request (freedom of information request act) if it will help you actualise and realise the weight of our grievance and concern that I have now formally enlightened you to.

Hi DWP

this is just a small plea to let you know I have been trying to get my local Redditch job centre to acknowledge our formal complaint for three weeks or so now, regarding their GPs evaluation of my partner emma jane Goode’s current chronic health condition. They have not responded at all to the formal complaint.

 

we are both accusing the GP of making things up or lying to be very honest with you, and my partner and i have already formally complained to the Redditch job centre in writing multiple times, and also now insisted they acknowledge the formal complaint as an FOI request where ever a question is asked or present. (FOI act freedom of information act)

A member of staff has also re-evaluated Emma since, and totally and utterly ignored our complaint. Initially the job centre even refused our paperwork when they insisted Emma come in, and universal credits were the ones that asked us to supply it in the first place!!!

 

It was posted recorded delivery on Tuesday 14th December 2021 to get it into Emma’s file, on the very same day, and they still haven’t confirmed receipt of this. Even when asked for confirmation through the online journals since too.

 

I even asked how I can make a formal complaint, and got no response whatsoever. Thank god for your wonderful website, and this is how and where I got your address, and why you are reading this now.

All recent info regarding Emma’s poor health has been included in this envelope for you to consider. There is much more has been posted to the job centre, as well as all out info posted on the journals.

Please please please can you help us to get some sort of response regarding our grievances, or at least advise us on where to go to next to get our complaint heard fairly.

3rd Letter To Doctors Tramadol

This letter is an update from the previous letter that I sent by recorded delivery on Tuesday 11th January 2022. I have received an email from St Stephens Medical Partnership on Tuesday 18th January 2022 saying that you are currently investigating my last letter I sent to you.

I sent an email to PRESCRIPTIONS, M81001 (NHS HEREFORDSHIRE AND WORCESTERSHIRE CCG) on Friday 14th January 2022 at 8.48am for a new repeat prescription of 60 Tramadol.

Later that day I received a text message from the surgery at 12.31pm which stated and I quote:

"Dear Mrs Goode

With regard to your recent prescription request, the GP has requested you contact the surgery by telephone only to book a GP telephone consultation to dis cuss this matter further.

Thank you, St Stephens Medical Partnership"

So I phoned the surgery at 1.30pm to book a telephone consultation about my recent prescription request. I was first told that I can have a telephone consultation on Monday morning. I explained how I thought it was about my recent prescription request and I needed a call this afternoon. I was then told a doctor will phone me between 3 and 4pm.

A doctor by the name of Dr Whasir phoned me at 3pm. She said that she was a locum doctor. Below are the notes from the telephone conversation we had:

Dr Whasir asked me what the call was for and how she can help me. She told me that she was told just to ring me and had no information on what the call was about.

I explained to her about the email I sent in the morning requesting for a new repeat prescription of 60 Tramadol. I then told her about the text message I got at 12.31pm.

Dr Whasir asked why I needed the Tramadol. I told her that I got Endometriosis and that I needed the Tramadol for the constant excruciating crippling stabbing pains I suffer with all day every day.

Dr Whasir told me that my last request of 60 Tramadol that I requested on Tuesday 4th January 2022 was rejected. Can you please tell me why this was rejected? NEED A REPLY TO THIS QUESTION.

I explained to Dr Whasir about all the hassle I was having with my Tramadol. I also explained that I needed 60 Tramadol and not 30 Tramadol.

Dr Whasir asked me how long 30 Tramadol would last me. I told her a week.

Dr Whasir then asked me how long 60 Tramadol would last me. I told her properly about 2 weeks.

Dr Whasir then asked how many Tramadol I take in a day. I told her sometimes its 4 but lately it has been 5.

Dr Whasir said she was working out the amount of Tramadol I take in a day. She then said that I was right about the amount of Tramadol I should be having and that I should be having 60 Tramadol and not 30 Tramadol.

Dr Whasir told me that she put my prescription back up to 60 Tramadol and sent it over to the Pharmacy. She explained that it will be ready to be collected on Saturday morning.

Dr Whasir explained about the side effects. I told her that out of all the medications I have had in the past, Tramadol is the only one that has never gave me any bad side effects.

You have now got an actual doctor stating that I should be having 60 Tramadol every time not 30 Tramadol. I would like this noted on my file and on your records for future reference.

Please can you make sure that my prescription of 60 Tramadol remains at 60? Unless we have a consultation and we both agree to reducing the amount down.

My partner Stuart went to the Pharmacy on Saturday 15th January 2022 and collected my prescription. I did receive my 60 Tramadol.

I would like to say a big thank you to Dr Whasir for her co-operation and understanding, as well as confirming and acknowledging that 60 Tramadol is a reasonable accepted amount for my current status and condition.

I would be very grateful if you can reply within next 7 days to both this letter and my last letter. Thank you for sending me an email telling me that you looking into my concerns regarding my Tramadol.
